These routes open on app.ministrium.com without signing in. Replace <slug> and the identifiers with your church’s values.
The forms for people only send details to the church: Connect Card, Requests, Formation enrollment, SMS, Group Finder, and the kiosk guest registration. They do not create a Ministrium login account. Every account is created or invited by the church.

Before sharing a route, confirm the slug, campus, or class and test the link in a private browser window.
Public forms
Connect Card
The page shows “Welcome to” followed by the church’s name and the Connect Card, with an ES/EN language selector (“Fill in your details so we can get to know you”).
- Fields: First Name, Last Name, and Email (required), plus a phone number.
- Checkboxes:
- “I agree to receive SMS messages from the church (optional).” is optional;
- “I agree to share my information to be contacted by the church.” is required.
- Submit: the button is Connect. After submitting, the page shows Thank You for Connecting!.

Requests
The Requests form has the subtitle “Fill out the form and our team will follow up” and an ES/EN language selector.
- Required: First Name, Last Name, Email, Phone, and Request Type (Child Presentation, Marriage, or Pastoral Counseling).
- Optional: Campus (if the church has more than one), Details, and the SMS checkbox.
- Submit: the button is Submit Request. After submitting, the page shows Request Submitted!, with Send Another Request.
If the church has not turned on public requests, the page shows Form Unavailable.
Formation enrollment
The form says “Enter your details to enroll in this class”. All fields are required: First Name, Last Name, Email, and Phone.
- Buttons:
- Enroll;
- Enroll with my account, also shown if the person is already signed in as a member;
- Join the waitlist, if the class has already started.
- Messages:
- You’re enrolled!, You’re on the waitlist, or Already enrolled;
- date notices: enrollment open until a date, not open yet, or closed.
- No public enrollment: if the church has not turned it on, the page shows Enrollment Unavailable.

SMS
The Text to subscribe page says: “To opt in, text START or YES to the church number.” It shows the keywords and the church’s number, or Number pending if there is no number yet.
To agree from the web:
- Enter the Mobile phone and check the notice box.
- Depending on the church’s setup, agree with I agree to receive SMS, or with a code:
- click Send code;
- enter the Verification code;
- click Verify and agree. If the code does not arrive, use Resend code.
The final screen shows Consent recorded, with optional links to Visit the church website or Fill out the Connect Card.

Group Finder
It shows the church’s cell groups on a map, with approximate locations. The I want to visit button sends the guest’s contact details to the leader. See Appear in the public group finder.
Kiosk guests
On the kiosk (/kiosk/<slug>/<campusId>, with an operator PIN), use New Guest. First and last name are required; email or phone is required (validated email, E.164 phone).
